The survey asks questions about you, your health, your relationship with your providers, reasons you may have missed appointments or HIV medications in the past, questions related to the use of technology, and your use of drugs and illicit drugs. It will take about 30 minutes to complete. You may skip any question that you do not wish to answer, but we would greatly appreciate if you do not skip questions and complete the entire survey. There will be no cost to you and there will be no payments for participation. Your participation is entirely voluntary and there will be no negative consequences to you if you do not participate.
If you do choose to participate, all responses will be kept strictly confidential. The survey does not require entry of any information that could be used to identify you. To further ensure your privacy, we recommend that you complete the survey when you will have 30 minutes of privacy and that you close your web browser after completing the survey.
