Background: In France, men who have sex with men (MSM) are the only population for whom new HIV diagnoses have not decreased. Usually, behavioural MSM surveys provide self-reported HIV prevalence. In 2009, a new survey Prevagay was set up in order to estimate HIV sero-prevalence in this population, and describe sexual risk behaviour according to HIV biological status.
Methods: The cross-sectional survey Prevagay was conducted in 14 Paris gay commercial venues (bars, saunas, backrooms) during 6 weeks. Participants were invited to self-collect a fingerprick blood sample and to self-complete a questionnaire. HIV biological status and its knowledge were correlated with socio-demographics information, HIV self-reported status, substance use, number of sexual partners, sexual behaviours, and sexual transmitted infections (STI).
Results: Among the 886 MSM who both provided blood specimen and behavioural questionnaires, 17.7% (n=157) were HIV positive. Of these, 20% (n=31) were unaware of their HIV positive status, 62% of them had tested in the last 12 months and 23% never tested previously. If undiagnosed HIV positive were younger than other participants, they were comparable in terms of substance use and sexual behaviours to diagnosed HIV positive.

Conclusions: HIV prevention campaigns targe MSM with condoms promotion are always a necessity, and should emphasise HIV status knowledge.
