MTF transgender´s situation in France

Background: MTF transgenders situation in France. The monitoring and taking care of MTF (Male to Female) transgenders in France don´t respect Transgenders Rights. There are no laws for civil data changements of transgenders, it´s ownership of the State the only institution able to change it (sex and first name). The social, medical and juridical follow up of trans is inadequated. There is no free access to endocrinologists for hormones therapy. The access to hormones therapy and medical care concerning identity gender is available only to those that wish a "change sex surgery".This conditions encourage the hiden hormonal treatment without medical follow-up.
French Social Health Coverage push trans that want a mammoplasty to make before sex change surgery.
The médical staff dont have a good experience about silicon injections complications (tumours, necrosis, cancer) and correlations and reactions of hormones with antiretrovirals. The administration official forms don´t include the choice of transgender it´s limited only for male or female issues are provided.

Methodology and results: In 2007 among 820 (N= Total number) trans are followed in our institution. 697 trans (85% N) take female hormones, the majority without medical care. 558 (68% N) have recurred to silicon injections.Trans living with HIV usually don´t have counseling about complications of hormones and ARV. 98 trans (12% N) present HCV co-infections. The violence against trans is not well followed up by authorities. We have registrated 3 murders. 148 (18% N) trans had been agressed.
In France ARV is available to illegal status migrants trans.

Conclusion: Social discrimination and the french administration´s lack of specifical responses aggravates vulnerability of this minority (french and migrant trans) against AIDS prevention and HIV epidemy.
